Before you start
Do you have a CRN?
A Centrelink Reference Number (CRN) is required for you and your child to claim the Child Care Subsidy. This does not mean you need to be on Centrelink payments — it's simply how the government identifies you for services like CCS.

The CCS is available to all working families — you do not need to be on welfare to receive it. It can cover up to 90% of your fees, and families with a combined income of up to $530,000 are still eligible for some subsidy. Apply through MyGov before you start so you're covered from day one.

Your CRN & your child's CRN
Each person has their own Customer Reference Number from Centrelink — yours and your child's are both required.

Full name, date of birth, home address, school name, and class (e.g. 3B, K Blue).
Medicare card
Your family Medicare card number and your child's individual reference number on the card.
Emergency contacts
Two emergency contacts with names, relationship, and phone numbers.
Medical information
Any allergies, medical conditions, medications, or dietary requirements your child has.
Documents (optional — can be uploaded later)
Birth certificate and immunisation history statement. You can skip these for now and upload them after enrolment if needed.

Bank account or credit/debit card for direct debit. This is required to complete enrolment.

Your progress saves automatically. If you need to stop mid-way, close the form and come back — everything will be waiting for you.
Enrolling siblings? You only fill in your parent details once. Add each child in the same form — no need to start over.
